How much does it cost to enable HIPAA-compliant features on forms?

    I currently manager many doctor websites across many different states and I need to come up with a HIPPA compliant form solution.


    I see your pricing structure listed but I am not sure how I would fit within it? If I need the jot forms for many Doctors what are the costs? Are their extra costs for the business service agreement?

    Would you be signing a business service agreement with me our with my doctor customers themselves?

    I am eager to put together something as soon as possible please get back to me when you have a chance.

    Hi James,

    Thanks for reaching out to Jotform Support. Currently, only the Gold plan and the Enterprise plan have HIPAA Compliance Features. If you'd like to enable HIPAA features on your forms, there will be no additional costs. You can check out this page to know more about the costs and the other inclusions in each plan that Jotform offers.

    As for your other question about signing a business agreement, can you explain it a bit more? Are you referring to whether Jotform and the subscriber will be signing a business agreement upon upgrading the account? If so, there's no need for this, as you'll be asked to review and agree to our Terms of Use during the upgrade process. If that’s not exactly what you’re looking for, can you explain a bit more, so I can help you better?

    Regarding the business agreement.

    If I sign up for an enterprise account and use it for many individual doctors websites. Who signs the business service agreement with you?

    Is it each the individual doctors as they are the ones doing the actual business through the form?

    or

    Me, acting as a reseller continually managing only the functionality of the form. Not what's being communicated through the form.

    I personally have no interest or liabilities within the HIPPA - Doctor - Patient relationship so it would seem to me that an individual BSA would be needed with each Doctor individually.

    I have one doctor at the moment requesting a quote for a HIPPA compliant form on their website and to sign a BSA in regards to said form. That's what led me to you. Additionally once figured I would like to offer and/or mandate it to each of my doctor customers. Any advice is greatly appreciated.

    In general, Jotform will be the business associate but to learn more about Jotform Enterprise and business service agreements, you need to get in touch with the Jotform Enterprise sales team. You can contact them at this link. You might want to check out information about business associate agreements (BAA).
