Account suspended collecting CC information

I was going to use my Jotform to inform patients of their responsibilies and to collect information that would allow for billing them and their insurance companies for mental health services. Does this violate Terms of Service? My account was suspended.

Thanks for reaching out to Jotform Support. I checked your account, and it was suspended. It appears that your Sign Document is requesting Credit Card information, which is against our Terms of Service. To reactivate your account, we must delete the offending fields from your form. Please let us know if you want to proceed.

If you want to collect payment on your form, you would need to use one of the Payment Integrations in Jotform to collect payments through your form. Please refer to this detailed guide: Mastering payment form integrations with jotform.
