    Hello,

I am in need of a form to enable students to sign up for their activity/class choices over a 4 week program. Each week- they need to select their choice for a sport activity and their choice for an enrichment activity. Each activity needs to have a limit and allow no more than 15 students to register.

    So for example:

    Week 1: Sports options

    • Soccer (15 spots)
    • Basketball (15 spots)
    • Tennis (15 spots)
    • Volleyball (15 spots)

    Week 1: Enrichment options

    • Photography
    • Painting
    • Song writing
    • MakerSpace

and so on for week 2, week 3 and week 4....

OR if there is an option to allow students to select their top two choices for us to be able see not just that someone picked soccer and basketball as their top two options but to see more specifically, I picked basketball as my first choice and 2nd is soccer.

    Thank you!

    Chile

    Hi Chile,

A copy of my colleague's form can be found here. You can clone that form instead. Based on the condition my colleague set up on the form, the Gift Registry widget will only show after the 17th and will hide again on the 23rd.

Reach out again if there’s anything else we can help you with.

    Hello,
Per your message below, are you saying I will not be able to view the widget conditions set until 6/17 and today is 6/14? The form as is is confusing as I don't see how it allows someone to select a choice that has a capacity or qty limit.

    Hi Chile,

You can remove the Date Picker Option from the guide sent by Christopher earlier so the widgets will always show. The Gift Registry Widget limits each option to 15 slots only. You can check out and clone my demo form which display each week activity at the same time. Check out how the form works in the screencast below:
