Here, the second and third column become active only if the first column has "Yes" to any of the body parts. Meaning, if first column has "No" to Neck, then no need to fill the other two columns.

Hi abhiispa,
Thanks for reaching out to Jotform Support. You can use the Single Choice element for the rows and the Paragraph element for the questions in columns. Then, with the correct conditions, you can show the second and third questions are only shown when "Yes" selected in Single Choice. Let me show you how you can do it:
- In Form Builder, click on Add Form Element at the upper-left side of the page.
- Under the Basic tab, scroll down and select the Paragraph element to add it on your form.
- Click on Add Form Element again and scroll down and select the Single Choice element.
- Adjust the questions and settings as you need, and that's it.
- In Form Builder, click on the element you want to hide and click on the black Gear icon on the right side of the element.
- In the opening menu on the right, select Advanced at the top, and toggle ON the Hide Field option.
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
- Select Conditions on the left, and click on Add Condition.
- Select Show/Hide Field and adjust the condition.
- Once you're done, click on Save below to keep your settings.
After setting the conditions, your form should look like this:
