    Hi abhiispa,

Thanks for reaching out to Jotform Support. If you're referring to the Single Choice Entry field, unfortunately, there's no option to add an image to it. However, you can try the Image Choices widget or the Image Checkboxes widget and see if any of those works for you.

    1. In the Form Builder, click the Add Elements button.
    2. Go to the Widgets tab.
    3. Search for the Image word and try the two widgets.

Give it a try and let us know how it goes.

Hi, Actually the question needs to have an image, while the choices are simply text based single choice. Can you help me with that.
The image below needs to come after the question and the choices after the image.

    Hello Abhiispa,

I understand the request, but I’ll need a bit of time to check if it's possible. I’ll get back to you as soon as I can.

    Hello Abhiispa,

Unfortunately, the ability to insert an image as a sublabel on a Single Choice element is not an available option. While the feature you’re looking for isn’t currently available, rest assured we’re always working to add new services and features to Jotform.

I've gone ahead and escalated your request to our Developer Team. When or if it's developed depends on their workload, how viable it is, and how many other users request it. If there are any updates, we’ll circle back to this thread and let you know. As a workaround, in the meantime, you can use basic elements in the Form Builder that achieve your request. Let me show you how it's done.

    1. In Form Builder, go to your My Forms page.

    2. On the left side of the page, click on Create Form.

    3. Click on the Add Form Element menu on the left side of the page.

    4. Under the Basic tab, scroll down the list and add these elements.

    • Paragraph element where we will enter the questions.
    • Image element where we will upload the image.
    • Single Choice element where we will add the answers, we will just need its label.

Give it a try and reach out again if you have any other questions.
