Hi Mars,
I checked your form named 'New Customer Registration Form' and saw that your Dynamic QR Code Widget is not configured correctly, that's why it's not working. Let me show you how to set that up:
1. In Form Builder, click on the Wand icon on the right side of your Dynamic QR Code Widget.
2. Now, you need to enter a field name in to the Field label to encode section, this will allow your QR code widget to map data from a specific field, then, click on Update Widget.
3. Next, click on Settings in the orange navigation bar, then, click on Thank You Page.
4. Now, click on the green Add Field button and select QR Code from the opening menu.That's it. Now you'll be able to view your QR in the Thank You Page. You can also clone this Demo Form I created for you and test it to see how it works.

Hi, I can't scan the barcode. Which app or how to display the data by scanning the QR Code from the data inputted by the client? Thank you.

You can check out our QR Code Reader. You can scan QR codes from your phone as well.
