Jotform Smart PDF Forms on Starter plans

Is there a fee for using the Jotform Smart PDF Forms builder?

We were hoping to create some registration and medical registration forms that would allow us to save each submission as an independent PDF file. But also be available to download as an .csv or Excel spreadsheet so that we can use Mail Merge to create some "cards" with information pulled from each form.

We can either start with a PDF form that we have of the current registration and/or medical forms...or build the new forms from scratch. But what we need is an online form that can be filled out by our players, that can be view/saved printed easily as a PDF, but also available as a something we can download in a spreadsheet to create player medical information cards from.

We are trying to assess the best JotForm options for this and the costs we will incur to do s.
