HIPAA: Forms got locked out

    I can't seem to remove HIPPA lock. We are not a HIPPA entity. Please advise how to delete so that I can edit my forms again.

    Hi Amanda,

    Thanks for reaching out to Jotform Support. If you're not taking Personal Health Information, you just need to select the “NO, I'm not a HIPAA-covered entity” option from the HIPAA initiation modal in the Data section of your Account Settings. Let me show you how to do it:

    1. In My Forms Page, click your Avatar/Profile at the top right of the page.
    2. Click on Settings and in the menu on the left side, click on Data.
    3. Once you are on the Data page, you'll encounter the HIPAA initiation modal.
    4. Select the “NO, I'm not a HIPAA-covered entity” option.
