IP Banned and suspended Account

Hello,

I recently started using Jot form intakes for my organization to get intakes from clients and I created a draft form and shared with my team to collaborate and after that my account got suspended and I am quite sure that my account has been banned in error and IP is considered Phising which is not correct obviously, we are a legit business with offices in US and Pakistan. Pleaase share any information which is considered as phising and we can look to correct that.

Your account was suspended because you’ve got a form named Insurance Credentialing Form that’s breaking our Terms of Use by collecting login credentials.

I've deleted the offending fields and reactivated your account. You should be able to log in now.
