QR codes won't scan

On some of my form submissions, the generated QR code is rectangular and will not scan.  Any suggestions on how to remedy this or insight as to what the cause may be?

I have tested one of your forms and I can't seem to replicate the issue.

Do you have any add-ons or any pop-up blockers, that may disrupt the widgets functionality?
