Why is it is showing that my form is not available? I’m an authorized dealer for att/DirecTv.

The notice is showing because your form has an expiry date set up in the form settings. Refer to this guide to either adjust or remove the expiry date: https://www.jotform.com/help/274-How-to-Clear-Your-Form-s-Expiration-Date
