Hi,
I saved one of our forms as a pdf and did a test by emailing it to myself. I completed the form and then selected "Submit" at the bottom of the page. Nothing happened. When I tried to save the pdf with the information I completed it saved as a blank form.
We are looking at this option for a small minority of our clients to use. Can you let me know why this is not working?

I downloaded a Fillable PDF form of your form http://www.jotform.us/form/82135065486156 and I successfully submit it.
You can find my submission on the submissions page of your form.
Please note that you have to use Adobe Acrobat Reader or Foxit to fill and submit a Fillable PDF form. More about Fillable PDF forms, you can find here:

also attached the download that download and it is unable to submit. Not
sure if I am doing something wrong or downloading from the wrong place.

I assume that you created a Fillable PDF form by following the steps form this guide.
I did the same when I was testing your PDF form and the Fillable PDF form is working correctly on my end.
The file you originally provided is stored locally on your computer and we cannot open it.
How did open your Fillable PDF form? Are you using Adobe Acrobat Reader or Foxit? Like I mentioned previously, you need to use Adobe Acrobat Reader or Foxit to fill and submit a Fillable PDF form.
The Fillable PDF form will not work correctly if it is opened in a browser.
Can I Open My Fillable PDF in a Browser to Fill It out and Submit?

Ok. That is the correct way to download a Fillable PDf form.
But, how did you open your Fillable PDF form? Are you using Adobe Acrobat Reader or Foxit? Like I mentioned previously, you need to use Adobe Acrobat Reader or Foxit to fill and submit a Fillable PDF form.
