Unable to access form submissions due to HIPAA

    I am not an HIPPA entity. You have locked my client information. Please unlock so that I may retrieve my client data. I am a gym. I am not required to comply with HIPPA

    Thanks for reaching out to Jotform Support. I’m sorry to hear you cannot access the form submissions due to suspected HIPAA. I’ll do my best to help you out.

    I checked your Jotform account and there is no form locked due to HIPAA. Can you tell us more information and which form you are referring to or is it all the forms you created? It would allow us to understand what’s happening if you could take a screenshot and send it to us. We have a guide here explaining how to do that.
