Hello:
I want to send completed JotForms using my email address. I refereed to the page "How to Setup SMTP for a Form," but I find description confusing. At what point on the form building page does a user set up the SMTP? I've looked and I see nothing beyond the standard email field in the basic form elements section.

You can set up the SMTP at any point, it could be while you are building the form or after building it.
I checked your account and I can see that there is no SMTP registered yet. Please follow the guide (How-to-Setup-SMTP-for-a-Form) and create an SMTP. You can proceed to the instructions section:

Still having some issues. I tried setting it up, but it still shows username as "No reply" and the email as "noreply@jotform.com". Maybe I just don't have the patience, but it seems I followed the steps correctly. How do we learn the port number? Maybe that's where I'm going wrong?
I'm sending these emails to myself. Maybe that's causing the issue?

How are you testing the emails?
Note that if you're using the test email button on the email wizard, the test will be sent with the default settings.
I would recommend you to kindly test your emails by submitting your form with real submissions. Also, the sender email will be reset to the default one if the SMTP settings are incorrect and do not work.
You may easily check your sender emails by going to this link: https://www.jotform.com/myaccount/settings
Please check the sender emails section, there you will be able to manage your sender addresses.
How do we learn the port number?
You need to ask for this information to your email service provider.
Please try double checking the SMTP information you're providing and let us know if you are still experiencing the issue.

That's basically what I'm trying ... I designed one form so that I could fill it out, and then forward it to one individual. I found using JotForm easier than filling out his microsoft word form. But when I fill it out and forward it to him, it still says "no-reply," etc.

Basically ... I don't want anyone to fill out this form except ME. I don't want anyone to send anything ... I will send the completed form to other people. They will need to see my email address.

I checked your form and I can see that the email notification and autoresponder are using an SMTP as the Sender Email.
I did a test submission in your form. After that, I checked the email logs and it shows that the Sender Email (From) is (noreply@formresponse.com) in the Email Notification. Here's a screenshot of the email header:
This means that the SMTP configuration is incorrect. Please make sure that the SMTP details are correct. You can also delete the existing SMTP and add it back. You may refer to these guides:
How-to-Edit-Delete-Sender-Emails-on-SMTP-Settings
Also, there are 2 SMTP in your account that has the same details, please delete the other one.
