Can you create a form that will take you to another form depending on your response?

    If I create a form thtat will just list an age group, then depending on the persons choice it will take the person to the correct form for their age. Is there a way to do this?

    You can achieve this using conditional logic in Jotform. Here’s how you can set it up:

    Create Your Main Form:

    First, create the main form that lists the age groups. This form will serve as the starting point for users.

    Add a dropdown or radio button field where users can select their age group.

    Set Up Conditional Logic:

    In the Form Builder, go to Settings at the top.

    Select Conditions on the left.

    Choose and configure your desired conditional logic type based on the user’s age group:

    Show/Hide Field: Use this to change form elements’ visibility. For example, if the user selects an age group, you can show follow-up questions related to that group.

    Update/Calculate Field: Assign a value or perform calculations to form fields based on the user’s choice.

    Enable/Require/Mask Field: Toggle form fields’ “required” flags or implement an input mask conditionally.

    Skip To/Hide a Page: Use skip logic to jump to or change the visibility of a form page based on the user’s answers.

    Change Thank You Page: Display a custom message or redirect your form after submission1.

    Change E-mail Recipient: Override your default email alerts based on user answers.

    Redirect Users to the Correct Form:

    To take users to the correct form based on their age group, you can use the “Change Thank You Page” option in conditional logic.

    In the Form Builder, go to Settings at the top.

    Select Thank You Page on the left.

    Choose Redirect to an external link after submission and enter the URL of the form specific to the user’s age group2.

    Create Age-Specific Forms:

    Create separate forms for each age group (e.g., one for children, one for adults, etc.).

    Customize the forms with relevant questions and content for each age group.

    Test Your Interactive Form:

    Save your changes and test the form by selecting different age groups to ensure that users are redirected to the correct form.

    Hi Scott,

    Thanks for reaching out to Jotform Support. Yes, you can create a form that will redirect form fillers to another form depending on their response. Check out the steps and screencast below to see how it's done:

    1. In My Forms, click on the orange Create Form button on the left side of your screen and create your first form, which will serve as a source form.

    2. Now, click on the Dropdown element on the left side of your screen and add it to your form.

    3. Next, click on Edit Options and enter the age groups you want.

    4. Create a form for each age group, and click on the Publish button in the orange navigation bar, then, click on the green Copy Button to copy the URL.

    4. Now, click on Settings in the orange navigation bar, then, click on Conditions.

    5. Click on Change Thank You Page and create a condition similar to the below for each age group, and paste the URL you copied to the URL field.

    6. Next, click on the green Save button to save your progress.
