How to create a audit form that calculated a score

I am trying to create a Safety Audit form that allows my users audit their area.. Basically a employee would be presented with questions and answers would be Yes No or NA. 

I would also like the user to be able to submit a picture or notes for each question.. I would also like the form to calculate a "score" based on yes and no responses.. Basically the score of the audit.. 

Anyway, any assistance would be appreciated.

You can use form calculations with calculated value fields to have the Audit form with survey score.

I would also like the user to be able to submit a picture or notes for each question.. 

You can add the Upload field after each questions for uploading picture. For the notes, you can use the Long Text Entry field for it.
