I have a uniqueID field which is calculated based on values from three other fields.
This uniqueID field is also set-up in Form Settings for unique answers.
However, it still allows duplicate values to be input and submitted.
Please check and revert.

Please accept my apology for the delay.
I cloned your form and deleted all form fields and just retained the fields which you have used in condition to insert value in "unique Id" field. When I tested your form, it seems to work as expected. I was not able to submit the form with the same "Tour Code", "Name" and "Primary Email" value. Please check the screenshot below:
I also went ahead and tested your form and it seems to work just fine. Please check the screenshot below:
I would suggest you to please test your form again and get back to us if the issue persists.
