I inspected the website you provided but I was not able to find any JotForm form.
Please provide us the username or the email address of your Jotform account. You can also provide us a link of your form or the exact webpage URL where the form is embedded.
You can also contact your former employee and ask for the account credentials.

I am unable to find any account with that email address.
Do you know the account username? Or do you have a form link?
