Thanks for reaching out to Jotform Support. Please note that if you forget your access code, your data is considered lost. Since you forgot the access code, I highly suggest that you disable the form encryption so your new submissions will not be affected. Let me show you how to disable the form encryption:

1. In Form Builder, click on Settings on the top of the page.
2. Look for Encrypt Form Data and toggle it to No.
