Form: How to create boxes in the form?

    I got this from the templates and am wondering how they made the Name, Address and all the other boxes on the form?

    This can be done by choosing a template of your choice.

    3. In the form builder you can drag and drop form elements as directed in the image.

    I meant the actual style of this specific template. I know how to do a background image but the box around "Full Name" how did they do that?

    After choosing this particular template If you drag drop the form elements you would get the box around those form elements.

    For example I have cloned your form and added one element in my form.

    The form is using a theme, the box around the full name has been edited already in that theme using custom css.

    You can however, edit your name field also by using custom css but do note that you cannot replicate that theme by only using the Form Designer CSS.
