Populate a form using data from Google Sheets

eu terei um formulário que servirá como um cadastro de membros da nossa igreja. gostaria de saber se adicionando os dados das pessoas (nome, endereço, telefone, email) se é possivel carregar esses dados de forma semelhante ao que está na duvida 01.

onde:

um campo do tipo menu suspenso vai carregar todos os nomes da coluna nome que estará na planilha google.

e se seria possivel carregar para campos ocultos os dados de telefone e email

para ficar armazenado com as respostas do segundo formulário.

Now, coming back to your question, as I understand, you wish to use a Google Sheet column as Dropdown selections and have it as a unique identifier to populate the form using the same data from Google Sheets. Unfortunately, it's not possible to use Google Sheets as data source for dropdown selection, I've escalated this to our developers as Feature Request here. As for populating the form using Google Sheets' data, I'm afraid it's also not possible at the moment. I've also escalated this idea to our developers, but when or if it's developed depends on their workload, how viable it is, and how many other users also request it. If there are any updates, we’ll circle back to this thread and let you know.

Meantime, you may consider utilizing our Spreadsheet to form widget to populate a form using a static Excel file as data source. We have a guide to do that here.
