Form: scale rating from 0 to 10

I can't seem to create a nice looking Likert Scale question.

It is very important that it is an 11 point Likert scale 0-10

You could use the Classic Form or the All Questions in One Page Form layout.

Using the Classic Form you can edit the width or height through css or the Advanced Designer.
