Is there a way to add background music to a form?

Yes, it is possible. First, you need host your mp3 file and HTML file on your web server. After that, you need to use iFrame Embed widget to insert the HTML file into the form. I also suggest you check this thread.
You can use the audio tag in your HTML code to embed the mp3 file. Here's a guide: https://www.w3schools.com/TAGs/tag_audio.asp

Here is the form to which I'm referring ... https://form.jotform.com/80456750745159I ... I'd like to directly upload an mp3 that autoplays. I tried using a link to a song in my Amazon Prime music library (via a hidden text box), but it doesn't seem to work. Thanks.

I checked your form (https://www.jotform.us/form/80456750745159) and I can see that you inserted a link to the webpage and not a direct link to the mp3 file.
Do you have an mp3 file of that music? Please upload it to Google Drive or Dropbox and then share the URL so that we can find a workaround.

Thanks again! I do have the mp3 song (See You In September) on my computer that can be uploaded. Currently, my only online music storage is on Amazon. I'll investigate Google Drive or Dropbox to see what they're about.
