I'm sorry for the trouble caused to you.
I did check your account and did not find any issue with that. Your account is active and your IP address 89.101.121.132 is not banned.
I have cleared your form cache and that seems to have solved this issue. Your form is back and works as expected. I would suggest you to please test your form again and get back to us if the issue persists.
