is there a way when putting together a jot card form that I am able to allow user to add more than one participant and their age and then use their age to determine if they need to see the parent guardian section? We need to allow up to four participants (8 fields). When using the multiple answers element it allows us have four entries, but the alignment is off when it’s viewed on mobile device. Is there any other way?
Also is there a way to create a conditional logic on a widget that contains to data points?

Hello,
Thank you for contacting us. You can use 'Multi-line Question' in order to achieve your requirements. It allows you add 8 fields in 4 rows and works on mobile devices without any issues.
Please check and let us know if you need further assistance.

Thank You! That is what i was using, but when it displays on mobile devices, this is what it looks like. how do we get it to align properly? I shows fine on computer, but not on mobile. This would get confusing if not aligned properly.

Try either adding or removing sub-labels to all the fields. It looks like the sizing different between the ones with and without labels is causing the issue. If that still doesn't work, let us know and we will take another look.

It looks like they all have sub-labels. For some reason they do not show up when previewing the form. I have included two screen shots that show it in the build mode and the other in preview mode. what do you suggest?

The sub-label is moved in between the field input and the actual sub-label. When nothing is entered in the text box, it shows in the text box. Then when any sort of entry is made in the field, it is moved to below the input. When I checked on mobile, the layout was correct:
