Lately we've had reports from a number of users that the Phone Number fields on our form (which are mandatory) are not appearing.
We have been using the Country Code widget to capture the phone numbers.
This has been working flawlessly for over a year but now we are receiving these reports.
We are unable to reproduce this or find any pattern to it.
Do you have any advice?

Is your form embedded in a webpage? Please share the URL of the web page where it is embedded, so we can test further.
