I just received notice of a PayPal charge of $45 from JotForm. I have not used the service for well over a year - in fact, when I tried to log in just now it said I was successful, but the page would not load.
Please refund the charge and discontinue the service.

The reason is that the subscription was never canceled. However, I just did it for you and a refund was given.
