I'm not completely sure what you meant by a sliding scale or payment option. In case you're asking how to allow your form to collect payments, what you need is a payment field to be added on your form.

If all you need is some sort of Payment Authorization (e.g. charge your users at a later date), then your choices would be limited to Stripe, Paypal or Authorize.net.
