You can add an input table to the form and then select inputs as a textbox fields. I have checked your form named "Weekly Check Sheet" and seen that you've already managed it as seen below:
Also, you can embed PDFs or Spreadsheets to the form with our widgets as you can see below:
Please try and let us know if you need any further assistance.
Thanks.

You need to enter columns and rows one by one to the input matrix field as you can see in the screenshot below:
As you can see, the second line of the columns is not possible to enter into the input table.
As another workaround, you can prepare the table in Google Spreadsheets and then embed it to the form by using our iFrame embed widget.
I have created the example table in the Google Spreadsheet as you can see it at the following URL: https://docs.google.com/spreadsheets/d/1P3WV2Vzun8GyoqitTPTl4t6MVvaxz0AxxU7C09h_qKs/edit?usp=sharing
After, I have embedded it to the form by using our iFrame embed widget as you can see the screenshot below:
You can check the example form on my end at the following URL: https://form.jotform.com/73091061123949
I hope this helps. Please try these solutions on your end and let us know if you need any further assistance.
