We have a questionnaire titled Aurora Private Wealth Practice Purchase Questionnaire. However, it does not show when someone is filling out the survey. Why is there always issues with jotform. I have the image loaded in, and it shows in the background during the edit, but it does not on the preview or when someone is filling out the form using a link. This is supposed to be the background image for the survey/questionnaire.

Sorry for the inconvenience.
I checked your form and found that the background image is there and it takes time to load since the size of the image is 9MB.
For the slower connection, it would be difficult to the user to see the image.
Please see the screenshot of your form in my browser:
The solution for this, you need to resize the image and when you save the image, try to use "Save for Web" option in photoshop(if you are using this tool). This will minimize the size of the image.

I checked your form from several Browsers, Chrome, Safari, FireFox and the image loaded fine in all.
I loaded your form several times as well, and I wasn't able to replicate this issue. I checked the image and it is uploaded to our servers which is a way to do it.
If you're still able to replicate this issue please provide the Browser name and version in which you tested your form so we can try to replicate this issue.
