When I create a PDF of my form the form is not formatted correctly and words are being cut off or not included.  Can you please adjust the PDF to show all words in an appealing format?  Thank you!

I was able to reproduce the problem when converting your form into a fillable PDF form.

However, I could not reproduce this on a form with a default theme so it's probably an issue with your form since it is styled through custom CSS. This is affecting the display of the PDF.

So here I made a cloned version of you form https://form.jotform.com/72713786377974. I have made some fixes on the theme for the sake of the PDF output.

Please clone this form and then get the PDF of the cloned form. Here's how to clone a form: http://www.jotform.com/help/28-Cloning-a-Form-from-a-Web-Page

    Here's how it looks:

Please take note that not all fields are supported with the fillable PDF that's why some of them are converted into text boxes. https://www.jotform.com/help/312-JotForm-Fillable-PDF-Creator-with-All-Points#What-Happens-to-Fields-That-Are-Hidden-by-Conditions
