My form is not showing as an actual button on Facebook but it is attached to the 'contact us' button on the left hand side of my page. I already have an 'email us' button and actually expected this form to be accessible in a similar way to that, ie a big blue button that people can instantly see. Is there anyway to do this? The form I am trying to upload is for potential people to refer less fortunate children for a free birthday cake. No one will know it is there if its just on the 'contact us' section. Likewise, if someone does want to contact us, they'll be presented with a referral form rather than contact details.
If this is not possible, how do I remove it?

I checked your Facebook page and the form is embedded as the Contact us page:
And that is how it works actually. What you could do is edit the naming "Contact us" to something more related. Here is how:
1) Go Settings - Edit page:
2) Click Settings next to the name of the page:
3) Click Edit Settings:
4) And change the name there:
Or remove the page if it doesn't work for you.
