It is the notification email content that you are going to receive it to your email address when your users submit your form in order to be informed.
You are able to change its content as you wish if you do not like the auto-generated format.

Moreover, I have cloned your form named "UPDATED Work for Hire Agreement (Darlisa Hall)" on my side in order to check it. I have seen that the notification email content is too long compared to the form fields you have. I have fulfilled the form and submitted in order to receive the notification. I have seen that a lot of form fields are missing as seen the example below:
I think you have updated the form and the notification email did not being updated. I recommend you delete the notification email and recreate again in order to make its content auto-generated again.
