It would be difficult for me to assume where can I find the issue. Here's the list of what I think where you find the issue:
- email content
- sharing the form via social media
- bookmark the form

Your other question: instead of form.jotform.co put First Step Homes. I did go into the Settings and the Jotform Brand was unchecked.
