Nor is my question being submitted to the Forum...
I have made changes to my form and there is no update on the site using it.
I have waited several hours and no change...
Please help

Since you are now able to submit your question here in the forum, I presume that the issue is now resolved. Also, I have cleared your forms' cache. If any of the problems persist, can you try clearing your browser's cache?

Unfortunately, your assumptions are incorrect. Clearing cache did not work. Perhaps it's a CDN issue your end...

I checked your form directly and the agent email field is there: http://www.jotform.co/form/71631179215858
However, I see that it does not exist when viewing it at http://www.kbpiforms.com/. If you have placed the full HTML source of the form there, you will need to get a new source code again from the form builder since it will not update your fields on your website after you made your changes.
Can you tell us how you placed your form on your website?

updated the form recently and didn't need to cut and paste new code. Not
sure why that happened...

Glad to hear that the issue is now resolved.
If this issue happens again, wherein the embedded form is not updating, I recommend you to clear your form cache. Here's a guide: How-to-Clear-Your-Form-Cache.
