When I receive an email that someone has filled out the form, not all of the questions that have been answered appear in the email.

Are you referring to this form http://www.jotform.us/form/72028191511145? If you are not seeing all the field answer of the submitted data on your notification email, can you please try to create a new notification email and test if you will now receive the whole submitted data on the new notification email?
