I have added a logo to the last page of my form and it looks distorted when I complete/test the form.
What are the size limits/perimeters and acceptable file types to add an image to the form

Your image is fine, as you can see in your form:
It is just how the Preview widget is showing all fields, but you can fix that by inserting the below code in your Custom CSS field:
#previewContainer li>div img {
max-width: 405px!important;
}
And here is how it would look:

Hi Jocelyn,
Either way you copy/paste the code on your form custom CSS should work. I think your email just parsed the spacing on the code, but it doesn't affect the script function.
So you can just copy the code and it should still work.
#previewContainer li>div img {
max-width: 405px!important;
}
