Why conditional logic is not working properly?

I set up conditional Logic, show/hide and they aren't working. Any help would be greatly appreciated. 

Are you referring to that condition only? 

I checked your form and the field is showing fine:

Then I checked all fields involved in conditions:

And it looks like it is working.
