We are continuing to get reports from our filed agents that they are not able to upload photo files to our jot form. This issue has only occurred since late last week ,prior to that we were not having any issues. The form used to ask for the user to select a file for attachment , now the system is not allowing the upload and the screen looks like the attached jpg.
Some agents are reporting that the form is now asking them to take a photo instead of allowing them to upload a photo.

I was able to replicate mentioned issue when I tested your form on the webpage you provided. I was able to replicate it on your standalone form as well.
I have then cloned your form, but I was not able to replicate same issue there. On my cloned form Take Photo widgets are working properly. Here is my cloned form:

I have cleared your form cache.
Please update the widgets on your form, to see if that will resolve your issue.

files to our form.
Most do see the "live" camera option now but that is difficult for some
users to make work.
Is there a widget that will allow a file to be attached to the form, this
is what we had on the form before something changed last week.
I have attached a screenshot of what we had prior and would like to know
which widget can we use to allow just file attachments again.
