Hi,
I'm having a similar issue because I am using jotform to build a mock-up of a website and used the words Username and Password. This form isn't actually going to be used by anyone; it's for design purposes only.
If I change the name of these fields are you able to unsuspend my account?

Hello,
I did check your form and found that you have added fields like "User & Password" & login button which makes it suspected of being used for phishing.
Authentication form can't be used in JotForm and creating such forms are against the Terms of Use. I have deleted the password field and activated your account again. Please note that you can use any other texts like "Key, Code"
Kindly refrain from creating form with email/password, login/password or credit card details.
