I'm not a computer guy. I believe I may have correctly built one form with a choice of two payment a business could use to make monthly payments. This form will plugin to word press so that a business can pull up on their computer. Did I do it right?
Thanks,

I have checked your JotForm and see that there is only radio field set on the form to select Dwolla or PayPal. Do you want to enable your users to make payments using Dwolla or PayPal? If so, you'll be required to create two different forms with Dwolla and PayPal payments separately.
Please refer to the guides below that can help you with setting up forms with Dwolla and PayPal.

Also, please take a look at the following guide that can help you adding the Dwolla and PayPal forms to the form that will be displayed on the web site.
