I have linked my form to a Google Sheet but it doesn't seem to automatically input the information correctly. It will work for a day or two and then it just stops filling out the Sheet. How do I fix this?

We had an issue earlier about delayed email notifications, but this is already fixed. Your notification email should be working fine now. If not, please try re-integrating the Google Sheets to your form. Here's a guide: How-to-Integrate-Form-with-Google-Spreadsheet.
Also, our Google Spreadsheet is currently working properly. I cloned your form and integrate it to a Google Spreadsheet and it is successful.
