I would like to take the following fields from this form:

You could create the customized link with the data passed from the first form, for example, here're the variables to capture the data from the first form fields:
select club - {homeClub}
select name - {input304}
select position - {position}
select email - {input306}
You can also follow this guide to get the form fields names: https://www.jotform.com/help/146-How-to-find-field-IDs-and-names
To receive the data in the destination fields you only need to use its name without the {}. Now do note that to pass the selected option from one drop down to another one they both must have the exact same options, otherwise the second drop down will not be pre-populated.
Here's a sample link passing data from the First Form to the New Member - Welcome Form:
https://form.jotform.com/70508391580962?homeClub={homeClub}&memberName={input304}&memberEmail={position}
Once you have the link you only need to set up the condition, redirect to the link once certain selection has been made. Here's a guide that will help you with that: https://www.jotform.com/help/317-Change-Thank-you-URL-Based-on-a-Condition

Also, it seems like it worked one time initially, when I tested the post data to the opening form, but then upon additional submissions it stopped working.

What do you mean by "additional submissions and stopped working"?
I believe this guide would also help you: https://www.jotform.com/help/351-How-to-Automatically-Pass-Form-Data-to-Another-Form. With that guide and using the conditional logic to redirect to a different form, then that would meet the requirements you have.
You can indeed pass the hidden fields to another form with the same way on how you do it on non-hidden fields.

What I'm stating is that after I created the post URL, it worked on the first test. However, after the first test submission it stopped worked. Please see the links.

Your form https://form.jotform.com/70504486114956 has text that includes the character "#" in the "select club" drop down field, example "Manassas #3142", that character is breaking the link which is why it is not populating your target URL's fields. I am not sure why it worked for the first time, though.
Try changing the character "#" with a complete word like "number" and see if that works.
