Hi, I created 4 forms but when I went to check on them, with the new upgrade it seems 2 of my forms are gone. I embed my forms since they are for websites, and I still have those but how do I get them back into jotform so when I need to make changes for the clients I can do this.

Hello,
Can you send us the URLs of your websites where you have embedded JotForms. Also you can send the form IDs of these form for us to keep track of them.

I've looked at the two forms embedded forms on the url's you've provided us above but both of those are google forms (which appear to be working on my end) and unrelated to jotform forms.
Unless you switched those out with forms from us that might've been there before?

We do apologize about that then and thank you for the update.
I've tried checking on this some more to attempt to get to the bottom of this problem for you but, unfortunately so far I am not finding anything to suggest otherwise yet.
When I checked your account I only found 3 forms currently so perhaps the other wasn't saved.

Yes, as of checking, you have 4 forms in your account.
Perhaps the other was saved on a guest account (while you are not logged in when you created the forms) and was carried over to your account when you logged in afterward.
Or sometimes, we also forgot that we have forms moved in the archived folder or trash folder. :)

This agreement is entered into and between Tennessee Humane Animal League Animal Care and Adoption Center (hereinafter referred to as "THAL") and (thereinafter referred to as "Foster"). The Foster agrees to voluntarily and upon consent, temporarily care for a homeless animal(s) on behalf of THAL and to agree to the following regulations. **Please check beside each regulation below**

Fosterer understands that the foster pet is only temporarily in his care and remains the property of THAL. Any and all adoptions of the foster pet will be handled through THAL. Fosterer is encouraged to assist in this placement, but cannot make the final decision.
Yes
The Fosterer will make no decisions or actions regarding the foster pet's medical care (including euthanasia) without prior approval from THAL. All well and sick visits will be scheduled by THAL. In the case of an emergency, Fosterer will make every effort to contact his foster mentor and the Center before considering an emergency visit. THAL may not reimburse for unapproved emergency vet visits.
Yes
THAL will continue to provide preventative care (including vaccines, dewormer, flea and heartworm preventative) and medication for some minor illnesses diagnosed and treated by a THAL approved veterinarian. Fosterer is responsible for any medications, treatments or office fees for major illnesses or injury arising while in the fosterer's care.

The Fosterer shall provide the foster pet with humane care and maintain him/her in accordance with all current state, county, and municipal laws and ordinance while the animal resides with the Fosterer. The Fosterer agrees to provide the foster pet entrusted to his care with adequate food, water, shelter, affection, socialization and exercise as he would his own pets.

The Fosterer shall provide the foster pet with a fully fenced yard or other humane means of exercise. The foster pet shall not be kept constantly tethered or constantly outdoors. Foster dogs shall be leashed at all times when outside if there is no fenced area available. Foster cats shall be housed exclusively indoors. Fosterer shall keep identification tag on the foster pet at all times.

The foster pet shall reside at the Fosterer's address. If the Fosterer cannot keep the foster pet, the Fosterer shall notify THAL immediately. The foster pet shall not be sold, abandoned, relinquished to a shelter or to another rescue group other than THAL. Fosterer will not relinquish custody of foster pet to anyone other than THAL. And no person under the age of 18 shall attend to or care for foster pet without direct parental supervision.

The Fosterer understands that THAL and its representatives have limited information regarding the temperament and habits of the foster pet. The fosterer will take all reasonable precautions (through the use of crates, leashes, supervision, and common sense) to ensure that the foster pet does not run loose, become a nuisance, destroy property, injure or be injured by another animal, or injure or be injured by another animal, or injure or be injured by any person persons.

The Fosterer does hereby release, discharge, and hold harmless THAL and any person, firm or corporation charged or chargeable with liability, their heirs, administrators, executors, successors, and assigns, from any and all claims, damages, costs, expenses, loss of services, actions, and causes of actions, whether known or unknown, belonging to the said Fosterer due to any action or occurrence from the date of this agreement arising out of or in connection with the care of the foster pet.

Fosterers must agree to an initial and periodic inspection by THAL. The foster pet remains the property of THAL, and can be removed from the Fosterer by THAL or designee at any time and without any prior notice.

In the event that the Fosterer wishes to permanently adopt the foster pet, an Adoption Questionnaire must be filled out and approved by an Adoption Counselor, and, if approved, and Adoption Agreement must be signed and submitted along with the appropriate adoption fees.
