Hi, it looks like my account has been auto suspended. Can you please re enable my account.

I checked the account associated to your email address sukhavasi92@gmail.com and the account is indeed automatically suspended because of your form - http://www.jotform.us/form/70280493243150 violates our terms of use. Your form contains username and password field which is against our terms of use.
I have removed the username and password field in your form and activated your account. You can now use your account and your form is now accessible. Please do avoid adding/collecting sensitive information such as username and password on your form so your account will not be suspended again.
