Hi,
I need to create a form to collect feedback on numerous workshops (about 25 in one form). Something like this: https://form.jotformeu.com/70263902367355
However, if I used this method I'd have to manually duplicate and rename all "rating" and "evaluation" fields + manually create conditions to show/hide rating and evaluation of only those workshops that were picked in the drop-down.
Is there any way how to make it in one field? Checkbox/Dropdown + rating + textbox so that the respondents would only evaluate checked/picked answers?

If you want to collect everything through a single field, you may be interested in using our Configurable List widget:

Your users would then simply add as many rows for as many workshops they need to fill out. For example, this is how the Configurable List widget could appear on your form:
