The card you have been using has been cancelled. The employee is no longer with us.
How do I give you a new card number?

Since you're using Stripe you only need to go to the billing section in your account: https://www.jotform.com/myaccount/billing
Once there click on the "Change Credit Card" button and it should open a pop up where you can enter the new cc info:
