I did a test submission in your form and it was successful. I was able to upload a PDF file in the file upload field.
May I know what browser are you using? I can verify that this is working properly in Google Chrome and Mozilla Firefox.
Please try disabling your browser add-ons or extensions. You may also try using another browser just to check if it will work or not. Lastly, you might need to clear your browser cache.
I have also cleared your form cache so that your forms will be refreshed from our servers.
Hope that helps. Let us know if you're still having issues. Thank you.

Are you using the form's direct link (https://www.jotform.com/63555024524957) in testing the File Upload field?
Is this form embedded on a webpage? If yes, then please provide the URL of the webpage where the form is embedded so that we can further investigate. You may also try re-embedding the form using the iFrame method.
I created a test form, please try clicking the "Choose File" button here. Here's the link: https://form.jotform.com/63556261468968. Also, try opening this form on different browsers.
Let us know if the issue is happening on the test form that I created. Please provide a screenshot of the issue if possible. Thank you.
