I was able to replicate the issue when I cloned your form. Please try to delete the current email notification and then create a new one. Please refer to these guides:

No one is sent, e-mail, the subject is no problem. But the message is gone. Thank you.
Please try to delete the current email notification. After that, please create a new one. You can just add the label manually in the email editor.
