I need a form that has a selectable drop down list of available fonts and an input text box that user can input message and another text field that shows message with selected font applied. Do you have a template or built in script that will do this.

I don't think we have a template that matches your requirement. However, this may be achieved by using multiple text fields to display the different font types with the entered text.
Please see the screencast below that can help you in adding the fields to the form.
Now, add the conditions to the form to display appropriate text field based on the font selected from the dropdown.
The form is now ready and it should be displaying the text as per the font selected.

Thanks again for this form but I am limited to the fonts available in the html editor. Is there a way to add to the fonts available. ie: Trajan Pro. Old English, etc.

Normally, the available fonts that you can use in the Text tool are the following:
